El osteosarcoma es el tumor maligno de hueso más frecuente en la infancia. Una de sus variantes el
osteosarcoma telangiectásico puede confundirse ocasionalmente con quiste óseo aneurismático. El objetivo de
este trabajo es comparar las características clínico morfológicas de estas dos entidades clínicas para evitar
errores diagnósticos en futuros casos. Para ello se presenta el caso de una niña de 6 años de edad, quien
consultó por aumento progresivo de volumen, dolor y limitación funcional en tercio proximal de pierna
izquierda. La radiografía simple de tibia y peroné izquierdo al ingreso, mostró lesión osteolítica, metafisaria,
radiolúcida y con bordes mal definidos. Histológicamente se observaron espacios quísticos con sangre,
separados por septos fibrosos muy finos. Ambos estudios se interpretaron como quiste óseo aneurismático. La
paciente evolucionó tórpidamente, siendo necesaria la amputación supracondílea del miembro. La valoración
retrospectiva de las biopsias concluye que el caso correspondía a un: osteosarcoma telangiectásico. Se resalta la
importancia de una adecuada correlación de las tumoraciones músculo-esqueléticas, que abarquen aspectos
epidemiológicos, clínicos, radiológicos e histológicos. La evolución de la paciente posterior a quimioterapia fue
satisfactoria y en la actualidad esta libre de neoplasia.

Resumen en otro Idioma | The Osteosarcoma is the most frequent malignant bone tumor in children. One of its variants that telangiectatic osteosarcomas may be confused with aneurysmal bone cysts. The purpose of this is to compare the clinic, morphological and imagenologic characteristics either these bone lesions, for to improve the diagnostic. A six year old female patient who consulted for progressive swelling, pain and functional limitation in the proximal third of her left leg. The diagnosis of aneurysmal bone cyst was made initially. The radiography of proximal tibia and fibula showed expansile lesion, radiolucent to the metaphysis. The histopathological study showed central cystic and/or hemorrhagic spaces and thin septations around these spaces. Both results were interpreted like aneurysmal bone cyst. The patient evolutioned torpidly and was necessary the amputation of the involved limb. A retrospective study of previous biopsies and the evaluation of the last available sample led to the final diagnosis of telangiectasic osteosarcoma. The importance of an adequate multidisciplinary correlation of musculo-skelethal tumoral lesions involving, epidemiological, clinical, radiological and histological aspects is pointed out in this study. The evolution of patient posterior a chemoteraphy was satisfactory. |
